A 2022 Politico investigation revealed that delays in discovering contamination often result in preventable illnesses and deaths. Many organizations fail to engage independent external partners for this analysis, which Coole attributes to poor workplace culture and management practices. Addressing these internal issues is crucial for preventing similar failures in the future.
